From PS to LR...

Sometimes when I save an image from PS to send it back to LR, the saved image goes right next to the original.  Other times, it goes to the end of all of my photos.  Does anyone know how I can be sure it always goes back near the original?  I can't figure out what makes it change locations.

This depends on what Sort order is set to on the toolbar at the bottom of the screen in Lightroom.

If it's set to Added Order, the image will be at the end.

Set it to File Name, and the image will be next to the original.

If you can't see the toolbar, press T.

Hi lauries,

According to the default workflow, the editing image should be saved next to the original image in Lightroom. You may try and reset the Lightroom preferences back to defaults and check if that helps.

Refer: Preference file and other file locations | Lightroom Classic CC and Lightroom 6

Mohit
