CS6 allows you to create PDF forms directly. For earlier versions, please visit the Acrobat forum for details about Acrobat’s form field recognition capability.

When you use the Form Field Recognition feature of Acrobat, it looks at the lines and turns them into rectangular form fields.
In InDesign, create rectangles, and then use the Buttons and Forms panel to turn them into form fields.
If you still want a line, put in the background, maybe on a separate layer.

Steve Werner May 25, 2012 8:12 AM (in response to rsukhram@tgioa.com)Just create a rectangle, select it and convert it to a Checkbox using the Buttons and Forms panel. Should definitely be a checkbox in Acrobat.
