You will have to write a custom script and specify exact field names and since you want to select specific selections, we will also need to know how to identify those selected items.
If a radio button or check box is not selected it has a value of "Off".
If you use the same name for a group of radio buttons or check boxes they will interact in a mutually exclusive manner. One selection will unselected all other buttons.
Once a radio button in a group is selected then it can not be unchecked. Check boxes can be unchecked.
I am a little confused. Did you mean "above the horizontal line" (first three rows) ?
i've grouped the radio buttons by:
GroupA: A.1; A.2; A.3; A.4; A.5
GroupB: B.1; B.2; B.3; B.4; B.5
GroupC: C.1; C.2; C.3; C.4; C.5
GroupD: D.1; D.2; D.3; D.4; D.5
GroupE: E.1; E.2; E.3; E.4; E.5
GroupF: F.1; F.2; F.3; F.4; F.5
If 1 or more of: A.1; A.2; B.1;B.2;B.3;B.4;C.1;C.2;C.3;C.4;D.1;D.2;D.3;E.1;E.2;E.3;F.1;F.2;F.3; is selected --> Result will be "not apt"
If non of: A.1; A.2; B.1;B.2;B.3;B.4;C.1;C.2;C.3;C.4;D.1;D.2;D.3;E.1;E.2;E.3;F.1;F.2;F.3 is selected --> Result will be "apt"
