I donn't know if you are using a desktop or a mobile phone or a tablet for this.
I have only implemented subcribe/unsubscribe to newsletter before with php on backend for desktops, yet some scenario like the following shoul work.
From your code I notice you are trying to send an image. Regardless of the attachment you need to send that to your server. server reads email adress adn attachment from your POST, or whatever method you use to send data. Once you have everything server side there should be some emailing libraries depending on the server scripting/programming language you use. You can assemble/create the email and send it using the libraries/classes.
Unfortunately I don't have code any code to share, so Google is your friend here ...

this is the way I tried. is this a right way? Actually I'm trying it on android phone and tablet. There is a image named "insurance_screen_shot.png" which I want to attach and mail it. Please guide.
