If you use InDesign CS6 to create PDF forms, you'll still need to use Acrobat Pro to do things like Reader-enabling the PDF file so it can be filled in.
If you use Distribute Form feature in Acrobat Pro, the filled out results will be collected into a PDF Portfolio. Using Acrobat Pro, you can export the results into a CSV file which can be opened in a spreadsheet or database.

I am looking out options to be done on the server side.
1. Create PDF form with data filled-in by retrieving data from database
2. Extract PDF form data and saves back into database when user submit the form
Could you please validate below possible solutions and let me suggest the better solution?
1. Adobe PDF library SDK
2. Adobe InDesign CS6 scripting (server side) - only to create PDF form
3. Acrobat Pro (as steve suggested above) - but not sure whether it can supported on server side
