I am creating a Facebook competition entry page.
I have created the graphic, but I want to place a form over the top. See the sample attached that I found online, I want to replicate this exactly with my own graphic and form. The placement for my graphic and form is identical. I just can't seem to find a way to lay the form in the same spot, or get it to work in any way. I've tried creating my own form in Dreamweaver, in Google Docs, and in a few random online websites.
Any help would be appreciated, I have been searching high and low for an answer and I seem to be going in circles!
Can anyone just give me a hint at least? So many people looking, no-one has any guidance?

Your posted picture tells us nothing about your code.
If you have not yet attempted to code it , a common way to achieve your form would be to set up an outer DIV container. Use CSS to size it as large as your image, and position it and then specify your image as a non-repeating background.
Inside the outer DIV, set up an inner DIV to hold your form elements, and position it with margins and padding as desired.
