Are you trying to set up the form such that submission notifications are sent to an email address in addition to your own? If so, please take a look at the following FAQ:

What I need to do is add a CC email "text field" to the form, so users can add an additional email address. We want to require that they fill out their department head's email when requesting a project so that the department head knows a project request has been submitted. And the department head is different for each request, so that's why an additional email address field that recieves a copy of notifications/summary of request would be helpful.
