- Using Max OSX v 10.7.4, 24GB RAM, 4.15Ghz hexacore CPU
- Premiere Pro CS6, updated to 6.0.2
- The RED files are just imported straight into Premiere. The ProRes files are in a Quicktime container. Everything is 25fps. The timeline is 1920x1080.
- I installed the "REDCODE CS4 Installer 1.7 Mac" from the RED website.
I tried removing all the effects, and it still renders the same giant blur.

The person who set up this project, for some reason, had the Width and Height under Video Previews in the Sequence Settings set to 4x4, instead of 1920x1080. So it was rendering only 4x4 frames, which is indeed very blurry when scaled up to HD.
Whew!
However, I've noticed that when I go to "New Sequence", all I see are DV, DVCPRO, DVCPROHD, and Mobile folders. Shouldn't I have presets for RED?

Deactivate Premiere Pro. Reboot. Launch and re-activate Premiere Pro.
If that doesn't work, then a full uninstall followed by running the Clean Script followed by a full reinstall should fix it.

Yes, you should. You've got a typical problem that I experienced too. I ended up reinstalling Premiere Pro, but your case might be different. Just follow the steps on this document:
