If you click on the "T" icon on the lower right of the Monitor panel, you'll create a blank title on your timeline.
Is that what you're looking for?
It almost sounds like you may be describing a DVD menu -- but it's not really clear.
What you have tried so far?
As long as you do not add Menu Markers to your timeline, you should get exactly what you're describing. Just select the Slideshow/Pan & Zoom template and replace the background with your photo.
I am not with the computer now until later tonight. I chose the tab that gave me the templates for the cover screen and the page menus. (The one to the right of "edit", I believe -- second from the left). I just couldn't figure out where to choose a blank screen. I ended up choosing the gray background with the minimal black writing on it has the template. And then, when I manipulated the word box and inserted my picture and put everything in the right place, and my preview showed my title in a word box and my picture centered in the middle, And the "click here" button on the right, once I burned to the DVD, my words were in the place I put them (albeit in a gray color -- the background was a black-and-white tone gray) and my picture was not there.

If you replaced the background for that DVD template with your photo, as you've described, I don't know any reason it wouldn't work. Sorry.
