Okay, sorted this one.
It was apparently a corrupt permissions and .db file.
I started by confirming that the 'PS_AppIcon.icns' resource file was okay (which it was).
Followed by running Disk Utility > First Aid > Repair Disk Permissions' via the Recovery Disk partition utilities > Reboot.
Then deleting the '.db' file in '~/Library/Application Support/Dock folder' > Reboot.
All instances of the icon corruption are now gone (e.g., Launchpad, Applications/Adobe Photoshop CS6 folder, smaller icon verions in finder frame and menus).
Note - the only thing I did previously was to run the Adobe Application Manager updates and install.

i am using imac and 10.8.2 2 or 3 days ago i installed 13.0.2 update. photoshop ran well everyday. closed this evening after working for hours.
now it will not start and shows a flash screen offering me PS6 as a trial. i also have a weird icon in my apps folder.
after estart the app icon is normal but cannot run the app.
adobe screwup, again.
i am going to uninstall, reinstall and update to 13.0.1. i will wait to see what happens.

I have been using and loving PS since version 4 and Illustrator since version 8, but CS6 of both will probably be my final purchase.
Will keep using this version until it (or need) has run it's course.
I simply can't afford to keep going down this path (enthusiast with no budget for creative cloud).
Lovely software when it doesn't create problems though. :)
A few other users have reported that rebooting the system fixed the icons.
Apparently there is some sort of problem with the OS icon cache - possibly due to the new version adding higher resolution icons.
