The playback doesn't start at all. It doesn't loop or stop after a short periode of time, it doesn't start at ALL...
I threw everything Adobe-related away with CleanApp + everything spotlight showed me. Restarted the mac and did a new instal of PPro.
Same thing... :/

That's one of only three fixes. Have you tried the other two?
I am having the exact same problem with Premiere Pro CS6 6.0.2 and MacOS Snow Leopard 10.6.8.
I have tried the first two recommended solutions:
1. Remove the plist file. No effect (other than having a new plist file generated as Premiere is launched.)
2. Enable Root user account and run Premiere temporarily as root. Limited success: Video playback (but no audio!) while running as root, but after returning to my usual user, the no-playback problem remains.
It is unadvisable to run as root as a matter of routine, and according to Adobe I shouldn't have to more than once, so I consider that solution to not have worked for me.
Solution #3 is to upgrade my MacOS. That could cause a number of headaches, and I've yet to see someone proclaim that it fixes the problem. In fact, I've found one forum post indicating a clean install of the latest MacOS did NOT fix the problem. So I'm very disinclined to try it.
Other things I've tried:
A. One user on another forum (Creative Cow) suggested saving a project to a network drive (not just an external drive) and relaunching with that project. That unusual solution worked for me exactly once, but the problem returned a few days later.
B. Downgrading my video cars... the bug only appeared for me shortly after adding a 2nd video card to my system (so I could have 3 monitors). Removing that card, and attempting the plist and root fixes as explained above, did not restore normal funciton.
I have filed a bug report with Adobe's online form earlier today. I hope it gets some attention. I see from various forums that numerous people are experiencing this problem.
