I am building a medical intake form for a doctor's office, and I have a section where I need to ask about allergies. The Likert Rating Scale field gives me what I need visually, but for the answers, I need patients to be able to select multiple answers on one row, which is not supported as far as I can tell.
Does anyone have a suggestion to get around this? Or an alternative? I really like the visual effect of the rating scale; the other options take up a lot more space and look rather clunky.
As far as a feature request goes, that would definitely be top of my list, along with being able to make multiple columns. I was told last year that Adobe was looking to include a multiple column option in a January update, but that hasn't happened as far as I can tell.
The rating scale can't support multiple selections per row. You would have to use the multiple choice field (though its not a table layout).

Too bad about the rating field; the multiple choice field is really ugly for that sort of question. I appreciate the response though.
