Means you have an out of date and buggy video card driver.
...And you'll want to visit the web site of the maker of your video card to get a fresh new driver. If it's an ATI card, make sure to get the Catalyst 13.2 beta driver (the released Catalyst driver version 13.1 is no good for Photoshop).
That second screen-cap scares me. Unless you are working on just such an Image, say for a client, I would immediately do a very deep anti-virus, and anti-malware scan.

I reinstalled the video driver and the problem seems to disappear.
