I tabbed through the preferences already. More specifically, which preferences are creating the version set with the duplicate photo on just some of the files imported?
Duplicates are not created on import unless you are also creating masters in the iPhoto library. Version sets are an option upon saving files after eaditing.

turned off automatically fix red eye and automatically suggest photo stacks.
Now it is importing without a version set which probably was an auto red eye edit. After reviewing the photos, in my opinion, the auto red eye edit does not work very well. Probably better to correct manually rather than have randon version sets upon import. Not worth the trade off in the organizer.

Yes, that's right. You figured it out. Auto red eye on import always creates a version so that you aren't stuck with PSE's odd ideas about what constitutes an eye.

LOL. I didn't like those red painted toenails in photo files either....
