Please make sure you inserted the page breaks within the right view, this is Web View (for HTML form) or Page View (for PDF form).

Will you be able to share the form to email@example.com, and also send me the genearated PDF file?
If this is not possible, then please attach screenshots and a little description on how you created the FormsCentral form.

We have taken a look at the images in your form and there seems to be an issue with the logo image that you placed at the footer of each page. I have updated your form with the correct image. I was able to see the logo on both the HTML fillable form as well as the generated PDF form. Please take a look and let me know if you have any question.
One more question, when you said that
"my boss opens it up and it appears differently, some of the page breaks are messed up."
Which platform/browsers are you and your boss using?
Remember that Page View (the PDF fillable form layout) page breaks are not related to Web View page breaks (HTML fillable form layout) This link should have more details: http://forums.adobe.com/docs/DOC-2655
Now, on the PDF form you may not see your footer at the same location on each page, the logo that you placed as the footer will appear just below the previous form field. This is because you are placing an image field as the footer. Formscentral currently does not support an actual footer.
Lastly, I would like you to email the footer image to firstname.lastname@example.org and also, how did you create this image?
I am having this same image problem. Do you know what they did to fix the problem with your image?
Actually they were not much help on this one. If this is the problem I was
thinking of; the image was outside of the margins so I actually inserted
another image over the other one and this fixed the problem.
