FormsCentral currently does not support automatic page numbering as you describe.
You may want to add or vote on feature ideas:
If you need to add a new idea click "Create an idea" under "Actions" in the top right.
Not having a full picture of your form, my guess is that you can try to add the page-number-separator (whether you add this by adding an image field or formatted text field as the separator, should not matter) on the top of your page, right after you add a page break.
Would you please explain why page numbers and header/footers are not available for documents that will NOT be used online?
For forms created with Acrobat, please use the Pages/Edit Page Design/Insert Header & Footer option from the "Tools" panel to insert header/footers.
If the form was created using FormsCentral as a PDF form, please take a look at this link on how to make a FormsCentral PDF form editable in Acrobat:
Once you can edit the PDF form, you can add header/footers using Acrobat.
