I checked all of the options provided in your link and I am still having the same issue.
The only thing I can think of is I had an active subscription, but since I continued to have this problem, I did go through the upgrade process, therefore I now have two subscriptions.
I tried to cancel the newest subscription, but it tells me I have to uninstall it to do so, and unfortunately, I don’t know what to do to uninstall the 2nd subscription.
I am losing patience – have a ton of work to do and have been playing with this for much too long – HELP!

Sorry, I'm confused. What do you mean by "uninstall"? FormsCentral is a web based application...
Did you buy Acrobat subscription or creative cloud subscription?
FormsCentral is not included in those subscriptions. It has to be purchased separately.
They are both Adobe FormsCentral Basic Service subscriptions...and I still am unable to create a form. I've used this product several times previously and have never had an issue in creating a new document until today
Basic only supports 5 forms (and buying 2 doesn't give you 10). The Plus subscription has unlimited form creation (and is actually cheaper - you just need to pay annual fee upfront, instead of monthly).
If what I stated above is correct then just contact support to cancel your current accounts and move to Plus.
