Using FormsCentral with a Paid account you can save your forms as PDF which can be filled out off line. The form has options to send you email notifications as well, so when the user does click "Submit" (online) the data is sent to the FormsCentral server and is available in your FormsCentral account in addition to being emailed to the address you had defined.
If you just want the form emailed on submit, and not have the data collected on the FormsCentral server, you could save a PDF that does not have the Submit button and in Acrobat you could add a Submit button configured to email.

I found this YouTube video tutorial using Acrobat XI, he covers the email button in about 2 minutes.
There are many others online but that seamed easy enough to follow.
