Extension Manager only installs/manages extensions for the corresponding version of products. For example, Extension Manager CS6 only installs extensions for Dreamweaver CS6, Photoshop CS6, InDesign CS6, etc. To install extensions for Photoshop 12.0.4 (CS5), you should use Extension Manager CS5.

Extension Manager CS5 should have been installed when you install Photoshop 12.0. If you can't find Extension Manager CS5 in your machine, you can download and install it from http://www.adobe.com/exchange/em_download/em50_download.html.

I'm not familiar with this process, so another question : as it seems there
is also an Extension Manager for CS5.5, should I
- Download it and install (in this case would my ormer extension still work
- "Update PhotoShop to CS5.5 (but it says I have the latest updaytes) and
Extension Manager to CS5.5

The Photoshop version corresponding to Extension Manager CS5.5 is 12.1.x. As you mentioned, you have installed Extension Manager CS5.5. So if you are sure you can update Photoshop to CS5.5 (actually the product name is Photoshop CS5.1 in start menu and the version is 12.1), you can use Extension Manager CS5.5 to install extensions for it.
