Sorry about the issue. Can you please provide the form url so I can investigate, I may need to do couple test submissions (you can send the form url to my email firstname.lastname@example.org). Also would it be possible to take a screenshot of the error, and ask respondents system info (what OS/browser is used) and at what exact moment the error shows up.

We don't save any information until the submit button is pressed (and "submission successful" message is displayed)

Sorry it's difficult to provide any additional information without knowing what kind of error was encountered. But it's rather unusual to see any errors during submissions.

Was the error specific to any devices (like android/iphone..) or OS (like Win or Mac) or browser (IE or Chrome)?
Can you please provide the form url, so we can test it on our side.

I've tried to do some testing (you probably want to remove the submission entry with "test" values), but could not reproduce the problem. Its possible that the error specific to certain browsers or environment. Hopefully we can get more info (error message, OS/browser) from Elizabeth and do more testing to understand the problem better.
