How do I either create or acquire cartoon speech-thought ballons for inclusion into Premere CS6 projects? Also, how to properly size them to fit?
Use them in the titler.
If you Google for text balloon font you will find much more examples.

I also like to create "thought balloons," similar to the second row, and then animate the little "circles/bubbles" to form the full "balloon."
These are just left over from my old site and are basically sized for DV.
I have never actually had a use for them but when I put out a request for templates and things, they showed up.
The advantage of using fonts is that they are vector based.

I would be inclined to use Ann's vector font
and animate them in Ae as Bill describes.
AE works well too.
For the animation of the "circles/bubbles," Motion>Scale, Motion>Position, etc., work well, along with Opacity, in my normal case.
Thanks so much for neatly solving me speech balloon problem. Unfortunately I don't know how to incorporate what I assume are fonts into my CS6 titler. Any help?
