FormsCentral is not HIPAA compliant.
You can read about the security of our service here: http://forums.adobe.com/docs/DOC-1384
On your first question, we recently added support to send Notification emails to any/all users the form is shared with, so we do support your request, but are not HIPAA compliant.
If you don't mind me asking, what makes it not HIPAA compliant? It appears that you can make it available to only people in your organization and it's encrypted.
I honestly don't know much more than that we are asked often, that we are not HIPAA compiant, and that the FAQ is what we have for security to share. As far as "only available to people in your organization", the forms can't be restricted to only people within your organization, anyone with the form URL can fill out and submit the form. You can share responses etc only with people you chooose, but the form is available to anyone who can access the URL.
Josh, I really apreciate your quick responses. Its good that you are asked about that, and upfont about it not being within regulation. The product that you have is very awesome, easy to use, and easy to distribute. Which is why people want to use it within their organization. I personally wish there was a HIPAA Compliant version, and or upgrade that you could purchase, that would be awesome.
Thanks again for all your help,
