What does the vendor say about their driver and PPro CC compatibility?

So everything should be covered on that front. And as I listed, I was getting playback out of AE CC, so I feel it is solely an issue in Premiere. That was worth checking out though since CC is so new. Thanks for replying.
So, you have playback to the external monitor now, I assume?
Still no. I had already downloaded the Blackmagic driver and tried to troubleshoot before I posted.
1. What media resoltuon and frame rate are you using?
2. Have you gone into Blackmagics control panel and set the 'Set default video standard as" to the correct output settings?

My sequence is 720p at 29.97. And I have tried the "Set default video standard as" at both 720 and 1080 at 59.94.
It appears to not be displaying only the 720 sequences. A 1080 sequence that I have shows up on the external monitor just fine, even when I have the "Set default video standard as" to 720 or 1080 in the Blackmagic control panel.

Turn the TV off for 10 seconds and then back on once you have a output signal set to 720P. Then minimize Premiere and maximize it again. I have already run into a syncing issue with a Blackmagic card and display today with CC.
