Can you provide more details on what you are trying to achieve. I don't quite understand what you are asking for.
You can't add buttons to the form in FormsCentral but you can add a Formatted Text element and insert links (Insert > Link...).
I am attaching the form I am trying to recreate for our purposes. My supervisor would like to be able to have the ability to include all the Partner/Agencies who participate with the districts for services in the same document, without have to create a separate form for each Partner/Agency. The buttons I am trying to include are highlighted in yellow.

I'm not quite sure what those buttons are supposed to do.
If they are supposed to show a different forms to fill out depending on the Partners then you can use show/hide logic and a choice field (e.g. Dropdown field to pick a Partner) to display the right fields for the specified Partner. You basically create a single form good for all Partners but only display certain elements depending on the selected Partner.
