You can import the PDF in FormsCentral and collect data with the service. You can either email the Submission Enabled Form to your contacts or post the PDF on your website for your contacts to download. Since you are working with a PDF the fillers will have to use Adobe Acrobat, Adobe Reader or Adobe Reader Mobile to fill out and submit the form.

If you want to your contact to fill out an HTML form then you will have to recreate the PDF form in FormsCentral (as opposed to import the PDF).

Go to the Distribute Tab and copy the Link if you want to distribute the web/HTML form or generate the Submission Enable PDF (paid account only) if you want people to fill out the PDF instead.
FormsCentral host the HTML form but doesn't not distribute it to specific people. You need to send the link (or the PDF) using your favorite email client... or you can post the link (PDF) online on your website.
