Download the PDF preview (which is not accurate to scale, but does have the dimensions listed).
If you tell me what version of ID you have, I'll set up the template file for you in ID. I started to tell you how to do it yourself, but the artwork is pretty complicated with the die line and it isn't symmetrical, so it turned out to be easier to just fix things.
Wow, that would be incredible! I have the CS5 version of InDesign. I guess my confusion stemmed from setting up the document - whether or not I needed to set up a doc that was 14.125" wide by 4.718" tall or what. Thank you SOOO much for your help!!!

Yeah, that's one scary place.
And using XPress with placed PDFs or any other placed art work with live transparency will mean:
At output it will be flattened. For generic XPress features with transparency this can be usually avoided by injecting special comments (PDF marks), but not with placed art work.
XPress cannot export PDF.*
(It's always PostScript or EPS under the hood, distilled to PDF)
