Can you confirm that FormsCentral application works that it is just when you try to see the fillable form in the web browser you get issues.
What is your Flash Player version?
Can you compare the versions with the browser that does work vs the one that it doesn't?

yes, Forms Central application works. I am logged in now and testing the forms.
Flash Player version: 11.8.800.94, Windows 7 (64-bit), Mozilla Firefox. I tested forms on different computers with different Flash Players with same issue.

Is this happening on different machine or just one?
Is this happening on mac and windows?
Is this consistent, meaning if it doesn't work on a specific browser it never works (as oppsed to sometimes it works, sometimes it doesn't)?
It is happening on different machines too. I have tested in on three different machines in our office.
Looks like it is happening only on Windows. On Mac its working fine. I have been testing in on MacBook Pro with Safari, Mac OS X, Version 10.7.5
Yes, it is consistent. Since yesterday it hasen't been working at all on Windows browsers.
Before that it was working fine and I didn't have any problems.

It might be worth to try outside you office enviroment. I can't reproduce on my side. I'm trying to figure out what could be different with your system and mine.
It is working outside our office environmnet. Our IT tald me it is issue with Firewall. We have standard Firewll set up which shouldn't be blocking it but it is!
If some Firewalls are blocking your forms there is no use. Interesting thing is that I have been having this problem only for last few days (we havent changed our Firewall set up for months).
Have you recently changed anything on your set up.
