Are you certain that your clip and all of your titles
match the frame size and aspect ratio of your Sequence?
Select a title, and check the 'Info Panel':

Be sure that your Sequence settings match your footage by doing this:
Or, right-click a media file and select 'New Sequence From Clip".
How do I choose the right sequence settings?
I didn't get the same output screens you show for Select a Title and Select a sequence, but I saw from the Properties Sheets that the sequences didn't match. I don't know why they didn't but I deleted the new Sequence and recreated it by dragging the intial sequence into the New Item icon like you showed and that resolved it, however I don't understand why that created a single video clip of the initial sequence.

When you create a new Sequence with a Sequence,
the result is a Nested Sequence.
Create a new, properly configured Sequence from a media clip instead.
Then you can copy / paste your edited clips into the new Sequence.

I didn't get the same output screens you show for Select a Title and Select a sequence,
I neglected to say "select a title (or Sequence) in the Project Panel"
instead of in the Timeline.
What version of Premiere are you using?
