What is the OS and version ?
What is the Graphics Card installed on the system ?
How much Ram do you have ?
Do you use any external Hard Drive ?
It will be helpful to know the info Sandeep requested. Other questions would be
1. Have you tried creating a new sequence from clip? Does the problem still occur?
2. Clear prefs? Holding down Shift+Option on launch.
3. If you have a GPU, does disabling resolve the issue?
4. Deleting your Media Cache?
5. Update to 6.0.4?

I'm using a MBP with OS X 10.8.3 and Premiere CS6 version 6.0.2. Graphics: Intel HD Graphics 4000 512 MB, 8GB RAM, 1TB USB3 external hardrive.
The thing I can't understand is why it did work but now doesn't. I experience the same problem if I create a new sequence from clip or if I create a new project. I've not yet cleared preferences or media cache, or updated to 6.0.4 yet...
Please try to check the same thing other / old footages in premiere pro.
and one more thing we need alleast 1 GB RAM for Graphics Card to work properly with Premiere Pro, that could be the issue.
but first check with the other footages also.
