What kind of PDF viewer are you using (Mac Preview, Chrome, others?) If you have not done so, please try viewing and printing with Adobe Reader/Acrobat or Adobe Reader mobile. And also please send the PDF form to email@example.com
Thank you for providing the PDF form. I am not able to reproduce your problem. When I printed the PDF form via Acrobat X or XI I do see all the fields including the radio buttons for the Yes/No questions printed.
Are you still having problem? Please try reinstalling your printer driver, or try to print to another printer.
