Can I generate text fields starting with a character style?

I'm working on a print document that will also need to be an interactive PDF. The document contains several worksheets like the one pictured below. The white boxes in this worksheet are created with a character style with an oversized white underline (23 pt wide, offset -7 pt)

How would you recommend I automate the creation of these form fields using the character style as the starting point, or should I just start drawing boxes manualy?

Note: The interactive pdf could be one large text field. It's not necessary for it to have three lines; the three lines are just a visual style for the print version. And, some fields are one and two lines large, but all are character styled "white_highlight".
