I have never seen this, and, as a developer, I rarely have a valid swf in the cache.
Can you give us more info? Client OS? Browser? Server? Application type, patterns? How do you know it is related to the cache?
We found some workaround that seems to work.
Maybe it helps you too ;-)
seems like it had to do with the web server. SWF caching was enabled. But the caching cut of the very last bits, so the app wouldn't start.
Disabling SWF caching did the trick.
