It's a little bit mysterious. Do you have installed the actual drivers and patches? It seems that the Intel HD Graphics 4000 is an internal graphic card. Have you tried to disable the internal card?

Yes, inte HD Graphics 4000 is an internal GPU, I have windows AERO turned off .. Windows should use it only for windows interface, GT 650m should be turned on when premiere is working.
Okay, I tried to turn Intel videocard off... I went to device manager and swithed Intel HD4000 off, my notebook screen turned black and I couldn't do anything , so I did restart and windows started with 800x600 resolution, I've switched it to 1920x1080 again and started Premiere Pro , where only HArdware mode was available and No GPU(cuda)... Also I tried to run Nvidia Control message and a message popped up"NVIDIA DISPLAY settings are not available you are not currently using a display attached to Nvidia GPU" . But in device manager it's turned on! Intel is turned off! )) I don't know how it works))
So I turned my Intel card on again and restarted windows, and all is the same again, GPU AND CPU are available in Premiere pro, but GPU mode shows BLACK SCREEN on playback
I have all latest drivers installed for both of video cards
Intel HD 4000 has 220.127.116.1167 driver
Nvidia GT650M has 18.104.22.16865 from latest 331.65 driver
I've reinstalled Windows and installed all up to date drivers for my hardware, all windows updates, net framework 4 and so on ... and installed premiere pro again
then tried to install updates for premiere pro one after another.. on version 6.0.0 and 6.0.1 my GT 650M is not listed as Cuda supported , so I have to install 6.0.3, where black screen appear when GPU is turned on, so it is on 6.0.4 and 6.0.5 ...
Can I get premiere pro cs6 6.0.2 update for windows ? Maybe someone has it downloaded? if this version really existed ))
Also render gives errors very often.

Thank you. Tried reinstalling intel hd graphics driver with one you've posted , no result.. will try reinstalling all drivers from clean system again .
FINALLY I'VE FOUND SOLUTION!!!
This problem appeared after updating Premiere Pro cs6 to 6.0.5. and Nvidia Graphics Driver to 320... and then to 331.65 . CUDA playback started working properly when I've reinstalled old 307.07 Nvidia Graphics driver. It was very unsupposed problem . NEVER UPDATE drivers and system till your projects are finished!
