You would create an attachment field. You could set the attachment field to only accept certian image file types (.png, .gif, .jpg). You could also create a Formatted Text field with instructions.

I tried that already. The links I need are time sensitive, and when I test the form, the page opens as expired and needs a new login. How could I try formatted text field? I tried that too, but it also just seemed to attach a link. I've been working on this form all month (as I just subscribed to FormsCentral), and the deadline for submission is today. I'm hoping my boss will like this form because it will eliminate the need for her to open so many web pages, but this is the only part I can't seem to get to work.
There is no was to just paste a screenshot in the fillable form. You would need to have people save their screenshot to disk locally first then use the attachment field to upload the image to the form.
Thanks Jeff and Genevieve. Now that I've created my form and I have included attachment fields, I need the form filler to be able to save the completed form for their records. I also need feedback of responses.
From my understanding, if I distribute it as a web file, it will not allow saving, or allowing them to finish it later. If I save it as a PDF, it won't let me attach files. So, what's the best way to handle this problem?
The good news is that very soon (in the coming weeks) we will support saving-work-in-progress with online forms. You will still not be able to easily print the online form but at least user will be able to start filling an online form and come back to it later to complete it and submit it.
You are right that attachment are not supported for PDF.

So, Can Adobe offer me anything that WILL help? I'm getting a little tired of purchasing these subcriptions because I'm told they will do what I need them to do, only to find out they don't. Now what? Seriously, there's no file attachment capability in PDF? You've got to be kidding. Please offer another way, then.
FormsCentral was originaly created to create online form. Attachments are supposed with online form.
We added the ability to save your form as a PDF but unfortunately the way that Acrobat and Reader submit attachments is not compatible with the FormsCentral service.
If you want to manage your data collection yourself you can create a PDF using FormsCentral (without a submit button) and then add attachment fields to the PDF in Acrobat. You will also need to add a Submit button to your form in Acrobat. You can set it up to submit the PDF to your email address including attachments. For help with Acrobat Forms you can look at the Acrobat form forum
