Do you mean the top row with the labels of all of your form fields? That should always be shown.

When I tested my form, a response table was generated but it did not have any headers shown for the data columns. Under the Forms menu, the "hide headers" option was listed but grayed out. How do I get the headers to show?
I started to input them manually by editing, but fear that they might not correspond with the data?
Any help you can provide would be appreciated?
Is the header row there but the cells are missing text? Is it possible you deleted them, that happens. If you select the table it will delete those headers (after a confirmation dialog).
You can check the History in the response table down in the bottom right corner and see if there was a time where the headers were there to copy that row and paste into the current version.
