What version of Acrobat or Reader is being used?
In Acrobat 9 you'd select:
Advanced > Security Settings > Digital IDs > Digital ID Files > Password Timeout (right pane) > [Enter Password]
and then change from "Never" to one of the other options.
In Reader 10:
Edit > Protection > Security Settings
to get to the Security Settings dialog.
In Reader 9:
Document > Security Settings

I can see the cert in the list of keychain digital IDs, but the Remove ID option doesn’t do the trick
In Acrobat, are you able to find out how to get the Security Settings dialog to come up?
Yes – security settings dialog comes up.
I believe the user hit “always allow” at some point – doesn’t seem to be a way to reverse this.
I explained how to change this in my first response. You have to select the "Digital ID Files" item in the panel to the left and the "Password Timeout" icon will show at the top of the panel to the right.

Yes, once you get to the Security Settings dialog, it's the same for each version. The trick is finding it.
User is reporting that Digital ID Files has nothing in it and so the Password Timeout is grayed out.
