I am not clear on what you are asking. Is this a question for an online/web form or for a PDF?
Are you asking to have responses sent to an email when the user clicks the "Submit" button, an "Email Notification"? If so FormsCentral does have an Email Notifications feature.
If I didn't get the question right please clarify.

I embedded the code for an online web form on the webpage but wasn't sure where the doc goes when someone hits "Submit". It is not PDF. I wasn't sure how the email notification connected to the form.
When a user fills out the form and clicks "Submit" the data will be sent to the FormsCentral server and will be available for you to view/analyze in your account, on the "View Responses" tab of the form. In addition, using a Paid level subscription you can set up "Email Notifications" on the "Options" tab of the form to have a notification email sent to you which can include the forms data (responses).
