Are you distributing as a web form or PDF?
The header will be shown for all pages of the form in the web/HTML form.
If you are distributing your form as a PDF the header is only shown on the first page. You could simply add the header image/text to each page of the form while designing for PDF, make sure you are viewing "Page View" so you can define the page breaks for the PDF and ensure that your "headers" are at the beginning of each.
I am distributing it as a pdf. How do you view it as a page view? I don’t see that option. Thank you.

Assuming you are signed into FormsCentral you would change the "View" setting from the "View" menu, or from the view selector at the bottom right corner when viewing the Design tab.
If you are not signed in and are using the Desktop application included with Acrobat Pro XI then you are already in Page View.
