Goodness. That's a hard one to answer without seeing the images. Some things you could try (depending on what version of Photoshop you are using.
Copy the layer and turn into a Smart object, because that's sensible.
Use the Camera RAW filter, and apply negative clarity. You could also try using the Camera RAW noise reduction using the Color sliders to best effect.
This will probably need some masking out to reclaim lost detail where you don't want it lost, but the Smart object comes with its own mask.
Or you could use the High Pass Filter to mask the important areas so you could use a variety of smoothing techniques on the rest of the image. The4 second part of this tutorial tells you how

Update! I'm pretty certain the reduce "noise" filter is what I've been looking for. Now to just play around with it enough to do what I want
