A jQuery Log-in will do nothing unless you have a server-side script to gather and process the form data. Which leads to the next questions:
- Which server-side programming languages are you planning to use for this?
- Exactly what do you want to do with the form data?
- What, if anything, will the user see once the form data is processed?

For the modal box, look at jQuery Fancybox2.
For the log-in form, use this HTML code:
<form name="input" action="your_form_processing_script.php" method="get"> Username: <input type="text" name="user"> <input type="submit" value="Submit"> </form>
For all the rest, you're on your own because I don't do people's homework assignments.
