I am trying to modify a dynamic stamp to read something like this:

Each of the "X's" represents a checkbox that would be checked to indicate whether the plan was approved or not.

I have no real knowledge of javascript and would appreciate any help offered.  Also, if there are any tricks to making sure the file stays dynamic once it's saved, I would appreciate it.
