For issues with Activation, I would contact Adobe Customer Support (not Technical Support), and explain what has happened. They have the power to reset aspects, such as Activations and installs allowed.
Now, if you ONLY had your Adobe product (assume Ps) installed and Activated on one computer, will only count as one, and the Adobe EULA allows two installations with Activation on two. However, I would still contact Adobe Customer Support, even if this would only be the second installation and Activation, as another crash could happen.
