Here is the link to the form - https://adobeformscentral.com/?f=cprxwZgO3dxIWEn05c6I%2Aw
And please see the attached email - please see the subject line.
I think if there is the attachment when they submit the brief, the subject line shows like that but not all of them show like that though..

Thanks for sharing me the link. I just tried to submit the form with test data, but I couldn't get email notification. If you get one, can you check if the notification email looks bad? Further more, can you please email me a copy of the attchment that caused problem that I can test with my own forms? I suspect the attachment may have caused the problem.
btw, I submitted below test data, please delete them from your response table:
roject lead contact name: wenlan
Phone number: 12345678
I deleted your submitted brief.
I can't pass the attachment to you as I am not sure whether I am allowed to.. (confidential issue maybe.)
It was the DOCX file. Do you think it might be the problem at our end? I just want to make sure with this as we don't want to lose any of details from the briefs.

If you can get email notifications from other briefs normally, your form should be ok. Is it possible for you to make a copy of the docx file with mock contents and send it to me?
Can I please remove the link of form and my work signature please?
