Yes, 5k is the max for all plans.

I am paying about $23 per month for my FormsCentral service. I am pretty sure that is a "Plus" account, but...how do I know? Where can I see it?

Yes. If you are paying $20 per month, then you have a Plus account.

Some of the info you got from the responses above is a bit incomplete.

5,000 responses is the limit for any one form. However, for safety's sake, there is a 2,500 buffer beyond the 5,000.

You should also consider exporting your records from FormCentral and into Excel as you get close to 5,000. You can store these exported records in Excel or a local database and then you can go back and delete the records in FormsCentral. So long as FormsCentral stays under 5,000 for any one form...then you are okay.
