FormsCentral should be running properly now, what are you experiencing? If you have not yet, restart your browser and sign in again, the service was recovered about 2:30pm PST yesterday. Let me know what you are doing and what errors you get, or signs it is not working.
I created a form on Friday, once I click on the FromsCentral email that I receive it usually takes me to the list of forms….its doing so but only showing them from 6/12/13, not the latest ones created…then said that I need to upgrade for additional responses….
Are you using FormsCentral only in a web browser? Or have you also used (or only used) the desktop FormsCentral application included with Acrobat Pro XI?
Do you have more than one email address you have used to sign into FormsCentral? Do you have a paid level subscription?
Just the web browser I believe…I just click on the link and then It takes me to this site https://formscentral.acrobat.com/app.html#d=dcVeEuot7QhjaebB68rh1w from there I download my responses into PDF format and send to the necessary individuals.
I have only use my email address but one other person is linked at the author, im the co author. We do have a paid subscription and haven’t had any issues until yesterday.
Who has the paid subscription, you or the author of the form? You are saying that after clicking that link (which is a link to one specific form in your account, probably from the email when the author shared the form with you), when you click the "X" next to the forms name after you've signed in the form will close and display a tab called "My Forms". Are you saying that you had additional forms in the My Forms tab that are not there now?
This table shows the features and # of forms one can be the "Author" of in the Free and Paid plans: https://www.acrobat.com/formscentral/en/features/compare-plans.html When forms are shared the features in the form (such as number of responses that can be collected, downloading PDFs etc) come from the authors subscription level. So, in a form shared with you by an author with a paid account would collect more responses than a form you created (if you had a Free level account), this could explain what you see as well as that if an author deletes a form or removes a co-author from a share it would disappear from your account.
Something certainly could be off, but I need to understand the situation better to figure that out or explain to you what you see.
Sorry may have worded that wrong…This is what’s email to me once a form is completed and submitted. Im assuming the author of the form has the subscription, but she no longer uses it. She has moved in the company and I have take up responsibility for the forms being submitted. Im not seeing the most recent submissions to this specific form.
Is there a way to change me to the author?

I'm still investigating a bit - it looks like the authors account should be at a paid subscription level but is showing up as Free level, I have forwarded the issue to a support team to look into further and will get back to you when I have more information. If you are in contact with that original author you might want to ask them to contact me about this as well.
I have all her log in info. She no longer uses the program and once the issue occurred I went in and updated the credit card info. Please let me know as soon as possible, thank you.
Can you tell me the aproximate date that you went in and updated the credit card info? We're still looking into why the account shows up as Free when it is a Paid account...
We've corrected the issue with the account level of the author account - things should be working as expected now.
