I am not sure if I understand your question. If you only allow your user to choose one option only, you can use Single Choice item in your form, then they can make one selection only. However if you want all of your options will be selected only once by all your users, for example, if user A has selected option1, then user B or C can no longer select option1, FormsCentral doesn't have that functionality.

I looked at your form and understand that you only want your users to make 5 choices in the grid you have at the bottom of the second page, but unfortunately FormsCentral doesn't support this functionality currently.
I'd like to do the same thing. I'm asking reviewers to select the best candidate for an award, and want them to rank the candidates in order (1st choice, 2nd choice, 3rd choice, etc.).
My thoughts were to either use the rating scale option or a series of drop-down boxes. For the drop-downs, the first one would say "Indicate your top candidate" with the drop-down listing the nominees, the second one would say "Indicate your next choice," etc. My concern with doing the drop downs is that sometimes mice slip, the scroll wheel jumps, etc.
I decided to use the rating scale, and just put "please use each number only once."
Did you find a different work-around?
