You have to create the button in the Interactive Objects tool set, not the Forms tool set.
Then you can go to the Forms tool set, Edit Form, right click on the button and duplicate across pages.
For some reason it shifted the button on me every 7 pages or so, but it was still better than pasting it and having to change the location on every page.
This stops the form header bar from showing up for some reason.
