Is this happening with your other forms as well, or just the one form? This may be a case of another browser plug in causing interference or perhaps there is a hung process. Is it possible for you to clear your browser cache then try logging back in? With your permission I could add myself as a co-author to the form and attempt to export the responses on my end and email them to you. Please let me know how you would like to proceed.

Hi Jeff, I cannot add you as a collaborator, when I right click and select add collaborator i get the same lock up as image 12, as it tries to open the form.
If I create a new form, that form is accessible, and I have tried many computers and browsers.

I was able to extract your responses to an Excel file and have emailed it to you. I am able to reproduce the issue you are seeing. This will require further investigation from our developer team. With your permission I would like to use your file in a bug defect report. Would this be acceptable?
