This may be a case of another browser plug in causing interference or perhaps there is a hung process. Is it possible for you to clear your browser cache then try logging back in? With your permission I could add myself as a co-author to the form and attempt to export the responses on my end and email them to you. Please let me know how you would like to proceed.

UniSydney Jun 11, 2014 3:30 PM (in response to Jeff Canepa)Hi Jeff, I cannot add you as a collaborator, when I right click and select add collaborator i get the same lock up as image 12, as it tries to open the form.
If I create a new form, that form is accessible, and I have tried many computers and browsers.

This will require further investigation from our developer team. With your permission I would like to use your file in a bug defect report. Would this be acceptable?
