Are you opening the PDF in Adobe Reader, in Workspaces, or in some other tool/service? In Workspaces, there is no mechanism for adding comments to PDF files.

You won't be able to edit the PDF file in Adobe Reader XI, but you should be able to leave comments on it. See: PDF annotator | Adobe Reader XI - features
