2. Click "+"
3. In Applications, select Adobe Photoshop Lightroom 5
4. In Menu Title, enter "Edit Capture Time..."
5. In Keyboard Shortcut, type the keystroke you want.

What I don't understand, however, is how I was able to invoke just the right command? I mean I just selected Lightroom as the application. I didn't specify any command within lightroom for that shortcut to invoke.
Can I do this with plugin extras too? I tried creating another shortcut called "Exif Write (modified photo files)" but it doesn't seem to work.

Yes, but you need to type exactly three spaces before the menu title. For example, "<space><space><space>Exif Write (modifies photo files)".

I don't know for sure -- it's probably a LR quirk. You have to do that with all plugin commands.
